Year 9 Immunisations - Mop Up Session

 

Dear Parent/Carer,

Please see the letter below from the Immunisation team.

Please follow the guidance below:

• If you have not previously filled in the form, follow the link in the letter and submit.
• If you do not give consent for your child to have the immunisation the form must still be completed and no consent selected.
• If you have previously filled out a form but your child did not get immunised you do not have to do another form, they will automatically get immunised on the next visit.

We cannot accept verbal consent.

If you consent after the link has closed you may contact the SAIS team directly on 0121 466 3410.  School will not be able to do this for you.
